San Juan does have Uber, with a marked rideshare pickup zone at the terminal, and the official white Taxi Turistico stand runs fixed, government-set fares. Puerto Rico is US soil, so Medicare and most US health plans generally travel with you here the same as they would at home; this is not the Medicare-gap pitch you may have seen for a foreign island. The real, honest gap is trip cancellation and interruption, most concretely across hurricane season (June 1 to November 30, per NOAA's official Atlantic season). If a day out to El Yunque, Vieques, or Culebra ever needed an evacuation, that is also outside what a domestic health plan typically arranges on its own. The policy exists so a storm delay stays an inconvenience instead of the most expensive part of the trip.

Puerto Rico is a US territory, so US health insurance and Medicare generally work the way they do at home; check your plan's island network before assuming a gap exists. Some credit cards already include trip-cancellation cover when you paid the trip with the card. Five minutes with your card benefits page and your insurer tells you what gap is actually left, so you buy cover once, not twice. What is usually worth pricing here: cancellation and interruption cover for June to November trips, when a named storm can reshuffle flights and ferry days on short notice.
